If you wish to pre-register at the practice you can do so by:
- Visiting the practice
- Visiting our Reception and Enquires room and complete the online form.
When you visit the surgery for the first time you will be asked to sign both forms to confirm that the details are correct.
Patients are required to provide a valid NHS number when registering with the practice. If you do not know your NHS number, please contact your current practice to obtain this.
Please also ensure that you provide a current telephone number for our records.
New Patient Checks
The practice needs all newly registered patients to consult with one of the Partners or a practice nurse within the first three months of registering. Medical Records can take some time to arrive at the practice therefore it is important for the practice to obtain as much medical information as possible about newly registered patients.
Patients who are on regular medication, attending the hospital for ongoing treatment, under the age of 5 years or over the age of 75 years, will need to consult with a doctor when registering with the practice.
Change of Details
Please let us know as soon as possible if any of your patient details have changed at all.
